Eyedentity Vision Care, LLC participates in most insurance plans. We are happy to bill for the care you received to any plan we participate in. Please be sure to bring all information regarding your insurance, including your insurance card, so that we can process your claim. Prior to your appointment, we recommend checking with your insurance carrier to determine your eligibility. If you have an insurance that we are not affiliated with we will provide you with all receipts you will need to submit privately.
